The anesthesia machine is meticulously divided into three essential operational areas:

  1. High-pressure system: Manages gas intake and initial regulation.
  2. Intermediate pressure system: Facilitates a stable and consistent gas supply.
  3. Low-pressure system: Fine-tunes the delivery of gases at the patient interface.

Within the intricate framework of the anesthesia machine, the high and intermediate pressure systems are precisely calibrated with pressure measurements in pounds per square inch (psi) or kilopascals (kPa), while the low-pressure system operates with pressure measured in centimeters of water (cmH2O). The high-pressure system, drawing from an E-cylinder with oxygen (2200 psi), air, and nitrous oxide (745 psi), employs a regulator to adjust the pressure to a safe 45 psi. This system is crucial during pipeline supply failures or in remote locations. The intermediate system, fed by the hospital’s pipeline at 50 psi, serves as the primary gas source in modern facilities, channeling gas to the flowmeters. Downstream, the low-pressure system administers a fresh gas flow (oxygen at 14 psi and/or nitrous oxide at 26 psi) to vaporizers, facilitating efficient volatile anesthetic delivery. This system supports the patient’s natural respiratory cycle.

Gas Flow Through the Anesthesia Machine

Outlined below is the sophisticated pathway of gas flow within our state-of-the-art circle system anesthesia machine:

  1. The journey of gas—comprising oxygen, air, and nitrous oxide—begins as it enters the machine via pipeline and cylinder supplies. The pipeline, typically the primary source, operates within the intermediate pressure range of 45 to 60 psi. In the absence of pipeline supply, the high-pressure E-cylinder system takes over, necessitating regulation to standardize pressure.
  2. In the intermediate pressure stage, gas travels to the flowmeter, empowering anesthesiologists to precisely adjust the fresh gas flow to the patient. Beyond the flowmeters, gas transitions into a low-pressure system, with pressures below 1 psi, measured in cm H2O (1 psi approximates 70 cm H2O).
  3. Gas may then interact with a versatile vaporizer, either bypass or measured flow, achieving the desired volatile anesthetic state.
  4. Next, the gas passes through a unidirectional inspiratory valve, ready to be inhaled by the patient via the inspiratory limb of the circle system.
  5. Once the patient exhales, the expired gas travels down the expiratory limb, flowing through an expiratory unidirectional valve.
  6. Should the ventilator be ‘switched off,’ or set to manual/spontaneous mode, excess system pressure is safely released through the adjustable pressure-limiting (APL) valve, with the released gas efficiently managed by the scavenger system.
  7. Any remaining gas within the circuit is then directed through a carbon dioxide (CO2) absorber, which expertly removes CO2 from the expired gas.
  8. Post CO2 absorption, the expired gas rejoins the fresh gas flow from either the pipeline or E-cylinders, enabling gas recycling—a process that supports low flow anesthesia and minimizes waste of valuable volatile anesthetics.
